Behavioral Health Home
Arundel Lodge’s Behavioral Health Home provides a coordinated, whole-person approach to care by supporting both physical health and mental health needs in one place. Our goal is to help individuals stay healthy, manage ongoing conditions, and improve overall wellness through consistent, connected care.
As one of Maryland’s first Behavioral Health Homes, Arundel Lodge partners with Luminis Health Anne Arundel Medical Center to offer primary care services on-site. This collaboration allows individuals to access medical care while staying connected to their behavioral health treatment team.
How the Program Works
Each participant is assigned a dedicated care manager who works closely with them to understand their health needs, coordinate services, and develop a personalized plan of care. This consistent support helps build strong relationships and ensures that individuals feel heard, supported, and connected throughout their care.
Our team helps monitor health conditions, identify gaps in care, and coordinate with providers to make sure all aspects of a person’s health are addressed. By focusing on prevention and early intervention, the program aims to reduce emergency room visits and hospitalizations while improving long-term health outcomes.
Who We Serve
Behavioral Health Home services are available to individuals enrolled in Arundel Lodge’s Psychiatric Rehabilitation Program (PRP). Individuals with a substance use disorder may also be eligible.
Program Focus
The Behavioral Health Home supports individuals in managing both medical and behavioral health needs through a comprehensive and preventative approach.
Whole-Person Care
We address both physical and mental health needs, recognizing how they impact one another.
Care Coordination
Our team works closely with medical providers, therapists, and support systems to ensure services are connected and effective.
Health Promotion
Participants receive support in building healthier habits, managing chronic conditions, and improving overall wellness.
Ongoing Support
A dedicated care manager provides continuous guidance, helping individuals navigate their care and stay on track with their goals.

- Goal setting for Individual Rehabilitation Plans (IRP)
- Periodic health and progress reviews
- Transitional care support after hospital visits
- Individual and family support services
- Referrals to community and social support resources
- Discharge planning when needed
